Job summary

Reform Rehab is seeking a Physical Therapist and/or Occupational Therapist to provide mobile outpatient therapy at Pegasus Landing of Chisholm Trail in Keene, Texas. The role focuses on seniors in assisted living, delivering consistent coverage and high-quality care.

Ideal candidates are dependable, patient-focused, with strong communication and documentation skills. New graduates are welcome, with weekday schedule options and leadership support available.

Qualifications

• Texas Physical Therapist or Occupational Therapist license in good standing.
• Graduate of an accredited PT or OT program.
• CPR certification preferred.
• Experience with older adults or senior living is a plus.
• Strong communication, organization and documentation skills.
• New graduates are welcome to apply.

Responsibilities

• Provide skilled PT and/or OT evaluations and treatments per discipline.
• Develop and implement individualized care plans.
• Treat older adults with mobility, balance, gait, ADL limitations.
• For PTs: therapeutic exercise, gait, balance, functional mobility interventions.
• For OTs: therapeutic activities, ADL training, functional training.
• Complete timely documentation and ensure Medicare compliance.
• Communicate with patients, families, facility staff, and Reform Rehab team.
• Coordinate care with PT/OT/PTA/COTA as applicable.
• Maintain a professional presence in the senior living community.
