Job summary

Reform Rehab is seeking a Physical Therapist and/or Occupational Therapist to provide mobile outpatient therapy at Pegasus Landing of Chisholm Trail in Keene, TX. You will deliver skilled evaluations and treatments to older adults in a senior living setting.

The role emphasizes dependable coverage, one-on-one sessions, and a weekday schedule with leadership support to grow with the company.

Qualifications

• Current Texas PT or OT license in good standing.
• Graduate of an accredited PT or OT program.
• CPR certification preferred.
• Experience with older adults or senior living is a plus.
• Strong communication, organization, and documentation skills.
• Reliable and comfortable working independently.
• New graduates are welcome to apply.

Responsibilities

• Provide skilled PT and/or OT evaluations and treatments.
• Develop and implement individualized plans of care.
• Treat older adults with mobility, balance, ADL, and functional limitations.
• For PTs: exercise, gait, balance, and functional mobility training.
• For OTs: activities, ADL training, and functional interventions.
• Complete timely documentation and comply with regulations.
• Communicate effectively with patients, families, staff, and Reform Rehab.
• Coordinate care with PTs, OTs, PTA, and COTA.
• Maintain a professional presence within the senior living community.
