Job summary

Reform Rehab is seeking a Physical Therapist and/or Occupational Therapist to provide consistent coverage at Pegasus Landing of Chisholm Trail in Keene, Texas. You will evaluate and treat older adults in a senior living setting, develop individualized care plans, and document thoroughly for Medicare and regulatory compliance.

Benefits include one-on-one patient sessions, flexible, predominantly weekday scheduling, and support from leadership.

Qualifications

• Current Texas Physical Therapist or Occupational Therapist license in good standing.
• Graduate of an accredited PT or OT program.
• CPR certification preferred.
• Experience with older adults or in a senior living setting is a plus.
• Strong communication, organization, and documentation skills.

Responsibilities

• Provide skilled physical therapy and/or occupational therapy evaluations and treatments based on the therapist's discipline.
• Develop and implement individualized plans of care.
• Treat primarily older adults with mobility, strength, balance, gait, and ADL limitations.
• For PTs: provide therapeutic exercise, gait training, and balance training.
• For OTs: provide therapeutic activities, ADL training, and functional training.
• Complete timely documentation and maintain Medicare compliance.
• Communicate effectively with patients, families, staff, and Reform Rehab team.
• Coordinate care with other therapy disciplines as applicable.
• Maintain a professional and positive presence within the senior living community.
