Job summary

Reform Rehab is seeking a Physical Therapist and/or Occupational Therapist to provide mobile outpatient therapy at Pegasus Landing of Chisholm Trail in Keene, TX. Coverage is focused on reliable, local hours with a one-hour patient slot and one-on-one sessions.

Qualified candidates will be licensed in Texas (PT or OT) with experience in geriatric care or senior living settings preferred. Flexible scheduling and leadership support are offered.

Qualifications

• Currently licensed Physical Therapist or Occupational Therapist in Texas.
• Strong communication, organization, and documentation skills.
• Ability to work independently and with care for older adults.

Responsibilities

• Provide skilled PT and/or OT evaluations and treatments as appropriate.
• Develop and implement individual plans of care.
• Treat mainly older adults with mobility, balance, gait, and ADL limitations.
• For PTs: include therapeutic exercise, gait training, balance training, and mobility work.
• For OTs: include ADL training and functional activities.
• Complete timely documentation and ensure Medicare/compliance requirements.
• Communicate effectively with patients, families, facility staff, and Reform Rehab team.
• Coordinate care with other therapy disciplines and maintain a professional presence.
