Job summary

Optum, part of UnitedHealth Group, is seeking a Nurse Practitioner or Physician Assistant for the Senior Living Program in Salem/Eugene, OR. The role focuses on home-based care in senior housing with an emphasis on integrated, high-quality patient outcomes.

Expect approximately 4–6 patient visits per day, with a non-fee-for-service model and strong inter-disciplinary collaboration. Applicants must hold an NP or PA credential, active license in the state, and board certification.

Qualifications

• Must have MSN or DNP for NP or PA degree and current active license
• Board certification required (AANP/ANCC or NCCPA) with applicable state licensing
• Ability to obtain start date license and DEA where applicable

Responsibilities

• Conduct comprehensive assessments and manage patients with acute or chronic conditions
• Coordinate care across transitions between care settings
• Document in CMS-compliant manner and ensure care plan alignment with facility staff
