Optum is hiring a Nurse Practitioner or Physician Assistant to join the Senior Living/Senior Community Care team in Salem, OR. The role emphasizes a life‑changing, patient‑centered care model without fee-for-service pressure, focusing on quality outcomes and meaningful patient relationships.

The position offers a balanced schedule with about 4–6 patient visits per day, extensive training, and a comprehensive benefits package.

Qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted Nurse Practitioner license in the state of residence or ability to obtain by start date.
• Board certified through AANP or ANCC with one of the listed specializations.
• Graduate of an accredited Physician Assistant degree program.
• Active and unrestricted Physician Assistant license in the state of residence or ability to obtain by start date.
• Board certified by NCCPA.
• DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive assessments.
• Manage patients through acute or chronic issues with testing, diagnostics, and interventions.
• Ensure accurate and complete documentation per CMS requirements.
• Collaborate with interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide counseling and education to patients and caregivers.
• Coordinate care during transitions across settings.
