Job summary

Optum Home & Community Care, part of UnitedHealth Group, is expanding its Senior Living Program to deliver home-based care in Multnomah and Washington County, OR. You will join a clinician-led team focused on high-quality outcomes and patient-centered planning.

The role targets Nurse Practitioners or Physician Assistants providing care in senior living and community settings with a supportive, non-fee-for-service model that emphasizes outcomes over volume.

Qualifications

• Graduate of an accredited MS,Nurse Practioner or Doctor of Nursing Practice program.
• Active and unrestricted license in your profession in the state of residence or ability to obtain by start date.
• Board certified by AANP/ANCC with applicable NP certifications.

Responsibilities

• Conduct comprehensive assessments and manage acute or chronic conditions.
• Document accurately with CMS-compliant records and ICD-10 codes.
• Collaborate with interdisciplinary team and coordinate transitions of care.
• Engage families and caregivers in care planning and education.
