Optum Home & Community Care is seeking Nurse Practitioners or Physician Assistants to join the Senior Living Program in Multnomah and Washington Counties, OR. Qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Graduate of an accredited Physician Assistant degree program.
• Active and unrestricted licenses in the practicing state.
• Board certification as NP (any eligible specialty) or NCCPA for PA.
• Active DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive assessments.
• Manage patients through acute events and chronic condition exacerbations.
• Ensure accurate documentation including CMS-compliant notes and ICD-10 coding.
• Collaborate with interdisciplinary care team.
• Discuss advanced illness and care planning with patients and families.
• Coordinate patient care across transitions between care settings.
