Job summary

Optum is seeking a Nurse Practitioner or Physician Assistant to join our Senior Living/Community Care team in Oregon. You will deliver high-quality care to patients in home and community settings, emphasizing value-based outcomes with no productivity requirements.

We offer a $30,000 sign-on bonus or $40,000 student loan repayment plus $2,500 relocation assistance. Expect 4–6 patient visits per day and strong interdisciplinary collaboration to support patients across care settings.

Qualifications

• Graduate of an accredited NP or PA program.
• Active and unrestricted license in the state of practice.
• Board certification appropriate to NP or PA role (AANP/ANCC for NP; NCCPA for PA).
• DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive assessments and manage acute or chronic conditions.
• Coordinate care across transitions and settings with CMS-compliant documentation.
• Communicate with interdisciplinary care team and families to plan care.
• Engage in advanced illness and end-of-life discussions as appropriate.
• Provide counseling and education to patients and caregivers.
