Job summary

Optum, part of UnitedHealth Group, is seeking a Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Salem, OR. The role focuses on home and community-based care with no on-call, no weekends, and no holidays, requiring at least 24 hours per month with a flexible schedule.

The Senior Living program prioritizes quality outcomes, interdisciplinary collaboration, and a patient-centered approach to complex care in community settings.

Qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted NP license in the state of residence or ability to obtain by start date.
• Board certification through AANP or ANCC with one of the listed NP specialities.
• Graduate of an accredited Physician Assistant degree program.
• Active and unrestricted PA license or ability to obtain by start date.
• Board certification by NCCPA.
• Active DEA licensure/prescriptive authority or ability to obtain post-hire per state regulations.

Responsibilities

• Conduct comprehensive assessments.
• Manage patients through acute medical situations or chronic condition exacerbations.
• Ensure accurate and complete documentation, including ICD-10.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide patients and caregivers with counseling and education.
• Facilitate agreement and implementation of the plan of care with facility staff and families.
• Coordinate care as patients transition through care settings.
• Review orders and interventions for appropriateness and response to treatment.
