Job summary

Optum seeks a Nurse Practitioner or Physician Assistant for the Senior Community Care team in Salem/Eugene, OR. The role focuses on home-based care for high-risk seniors within assisted living and community settings.

You will work in a non-fee-for-service model prioritizing quality outcomes, with ~4–6 patient visits daily to support patient well-being and relationships. Training is provided; flexible scheduling is offered.

Qualifications

• Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program.
• Active and unrestricted NP license in the state of residence or ability to obtain by start date.
• Board certified through AANP or ANCC, with certification in Family NP, Adult NP, Gerontology NP, Adult-Gerontology Acute Care NP, or Adult-Gerontology Primary Care NP.

Responsibilities

• Conduct comprehensive assessments.
• Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
• Manage patients during short-term sub-acute rehabilitation episodes.
• Ensure accurate and complete documentation, including ICD-10.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide patients and caregivers with counseling and education.
• Perform comprehensive assessments and document findings in a concise, CMS-compliant manner.
• Facilitate agreement and implementation of the care plan with facility staff, families, primary and specialty care clinicians.
• Coordinate care as patients transition through different levels of care and care settings.
• Review orders and interventions for appropriateness and identify the most effective plan of care.
• Function independently and responsibly with minimal supervision
