Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join our healthcare team in senior living facilities. You will deliver high-quality primary care, manage wellness, chronic conditions, and coordinate care with other providers.

This role involves on-site patient care across assisted and independent living communities, with travel to facilities as needed. You will contribute to patient outcomes and quality of life for seniors served by Curana Health.

Qualifications

• Must hold active NP or PA license in the operating state
• Master's degree in Nursing or PA studies required
• Strong leadership, interpersonal and problem-solving skills
• Excellent written and oral communication skills

Responsibilities

• Provide direct patient care and care management for residents
• Perform comprehensive assessments and develop treatment plans
• Conduct routine rounds and monitor residents’ conditions
• Coordinate care with facility staff, families and providers
• Document accurately in EMR and ensure regulatory compliance
• Participate in quality improvement initiatives to enhance care
