Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in senior living facilities. You will provide direct patient care, manage chronic conditions, and coordinate with other healthcare providers to improve health and quality of life for seniors.

This role requires licensed NP/PA credentials in the state of operations, board certification, and strong leadership and communication skills.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant in the state of operation
• Master’s degree in Nursing or Physician Assistant Studies
• Current board certification appropriate to role
• Strong leadership, interpersonal, and problem-solving skills
• Excellent written and oral communication skills

Responsibilities

• Perform comprehensive patient assessments, develop treatment plans, and provide ongoing care management for residents
• Conduct routine rounds to monitor residents’ conditions and adjust plans
• Coordinate care with facility staff, families, and other providers
• Educate residents and families on disease prevention and wellness
• Maintain accurate EMR documentation and ensure regulatory compliance
• Participate in quality improvement initiatives to enhance care delivery
