Job summary

Curana Health is seeking nurse practitioners or physician assistants to provide proactive, patient-centered primary care for seniors in skilled nursing and living communities. You’ll collaborate with physicians, care teams, and facility leadership to deliver high-quality, autonomous care with strong support from advanced technology.

Qualifications

• Valid NP or PA license and current board certification in your field.
• Master's degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal and problem-solving skills.
• Excellent written and verbal communication abilities.
• Ability to collaborate with physicians, clinical staff and leadership.

Responsibilities

• Deliver High-Quality Primary Care with comprehensive assessments and individualized treatment plans.
• Diagnose, treat, and manage acute and chronic conditions with preventive focus.
• Provide care management to improve quality of life and independence.
• Educate residents and caregivers on disease management and wellness strategies.
• Coordinate with care teams to develop and implement comprehensive plans.
• Maintain EMR documentation and ensure regulatory compliance.
