Job summary

Curana Health is seeking Nurse Practitioners or Physician Assistants to deliver proactive, relationship-based primary care to seniors in skilled nursing, assisted living, and independent living communities.

You'll practice with autonomy, collaborate with physicians and care teams, and use advanced technology to improve outcomes while reducing hospitalizations. This role requires travel to assigned facilities across our network.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.

Responsibilities

• Deliver high-quality primary care across skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with preventative focus.
• Provide ongoing care management to improve quality of life.
• Build meaningful patient relationships with residents, families, and care teams.
• Monitor health status and intervene to prevent complications and hospitalizations.
• Educate residents and caregivers on disease management and wellness strategies.
• Collaborate with physicians, clinical staff, and facility leadership to coordinate care plans.
• Maintain accurate EMR documentation and ensure regulatory compliance.
