Job summary

Curana Health, Inc. is seeking an experienced Nurse Practitioner or Physician Assistant to lead and deliver proactive primary care for seniors in skilled facilities and communities nationwide.

You’ll build meaningful patient relationships, manage chronic conditions, and work with physicians and care teams to optimize outcomes. The role offers autonomy, supportive leadership, and access to advanced technology and analytics to guide patient care.

Qualifications

• Masters degree required for nursing or PA licensure and board certification.
• Valid NP or PA license in the state of operation and current board certification.
• Demonstrated leadership, interpersonal, and problem‑solving skills.
• Excellent written and oral communication skills.

Responsibilities

• Deliver high‑quality primary care to residents in assorted senior living communities.
• Diagnose, treat, and manage acute and chronic conditions with emphasis on preventative care.
• Provide ongoing care management to improve quality of life and independence.
• Collaborate with physicians, staff, and families to coordinate comprehensive care plans.
• Maintain accurate EMR documentation and ensure regulatory compliance.
