Job summary

Curana Health is hiring Nurse Practitioners and Physician Assistants to deliver proactive, relationship-based primary care to seniors in skilled nursing and assisted living communities. You will work with a team of physicians and clinicians to develop care plans, monitor outcomes, and support patient independence.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Master's degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and verbal communication skills.

Responsibilities

• Deliver High-Quality Primary Care: perform comprehensive health assessments and develop individualized treatment plans for residents.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management to improve quality of life and residents’ independence.
• Build Meaningful Patient Relationships: conduct regular facility rounds and develop relationships with residents, families, and care teams.
• Monitor health changes and proactively intervene to prevent complications and unnecessary hospitalizations.
• Educate residents and caregivers on disease management and wellness strategies.
• Collaborate Across the Care Continuum with physicians, staff, and families to coordinate care plans.
• Maintain accurate EMR documentation and ensure compliance with standards.
