Experience Senior Living LLC is seeking a dedicated Care Partner to support residents with daily living activities and personal care. This full-time role offers day shifts (6am-2pm or 2pm-10pm) and opportunities to work closely with a compassionate team in Fort Collins.

Training programs are provided to ensure high-quality care and safety. The ideal candidate has a high school diploma or equivalent, with CNA/Home Health Aide preferred.

Qualifikationen

• High School Diploma or equivalent.
• CNA/Home Health Aide preferred, but not required.
• Medication Assistance certification as needed per state regulations preferred.
• CPR and First Aid certifications preferred.

Aufgaben

• Assist residents with personal care and activities of daily living as assigned and per service plan.
• Perform daily housekeeping tasks per established procedures and standards.
• Perform laundry duties as assigned.
• Review and participate in 24 Hour Reports, shift change reports, and verbal reports as needed.
• Assist residents to and from scheduled life enrichment programs.
• Promote resident choices, independence and spiritual needs as needed.
• Provide verbal encouragement and support to residents.
• Prioritize tasks to ensure optimum services as needs change.
• Communicate changes to the Director of Health & Wellness and to other departments as needed.
• Respond to resident pendant/wearable calls promptly.
• Ensure compliance with new resident care needs and infection control practices.
• Report accidents and incidents to supervisor.
• Maintain confidentiality of residents’ personal care information and respect their rights.
• Be prepared for emergencies.
