Sinceri Senior Living in Medina, OH is seeking a full-time Care Coordinator to oversee caregiver scheduling and resident care. You will document changes, communicate with families and staff, and ensure smooth operation of daily services in a resident-centered environment.

The role requires experience with healthcare or memory care settings, strong documentation skills, and the ability to lead during emergencies while maintaining quality resident care.

Qualifications

• Experience with elderly in healthcare, assisted living, or memory care.
• Shift scheduling experience preferred.
• Ability to document and report resident changes clearly.

Responsibilities

• Address family concerns and document issues with reporting.
• Manage minor facility issues and coordinate with Maintenance.
• Assign caregiver shifts and monitor performance.
• Schedule staffing and conduct ward tours.
• Communicate with physicians and other providers.
• Lead first aid, safety, and disaster response when on duty.
• Record and follow up on resident observations and orders.
