Job summary

Cogir Senior Living seeks a Resident Care Coordinator to be the liaison between residents, families, and the care team, ensuring a supportive, safe living environment. You will assist the Health and Wellness Director, schedule staffing, supervise team members, and help coordinate care with external providers.

You will support medication administration, address resident concerns, and help ensure regulatory compliance while delivering compassionate, dignified care for residents.

Qualifications

• High School Diploma or equivalent required.
• Valid medication administration license or state requirements for assisted living.
• An LPN/LVN license is a plus.
• Current First Aid and CPR or ability to obtain.

Responsibilities

• Schedule and assign caregiving and medication tech shifts for coverage and compliance.
• Participate in recruitment and interviews for care team members.
• Supervise, train, and support care team to uphold standards.
• Supervise medication room and orders with technicians and caregivers.
• Maintain accurate resident records and care plans.
• Coordinate on-site care planning with home health agencies.
• Coordinate admissions with sales and supervise move-ins.
• Conduct family/resident admission interviews.
• Facilitate communication among residents, families, and healthcare providers.
• Address residents' needs, requests, and complaints.
• Assist with medication administration and provide direct care when needed.
• Understand and apply community care regulations and state rules.
