Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team at a senior living facility in Reading, PA. You will ensure high-quality in-home care coordination, collaborating with clinicians, staff, and families to support patients with chronic conditions.

The role requires compassionate, organized individuals who can manage multiple care streams, document communications, and use EMR systems effectively. In-office, daytime hours, with comprehensive benefits offered.

Qualifications

• Must be comfortable with speaking on the phone for large portions of the day.
• Must be compassionate and empathetic towards patients, providing exceptional customer service.
• Ability to take accurate notes to document tasks in a timely manner.
• Ability to multitask between patients and workstreams while staying organized.
• Must be able to work full-time, Monday–Friday, daytime hours, in Reading, PA.

Responsibilities

• Complete individualized patient care plans and perform care management using the EMR system.
• Frequent contact with patients to provide care coordination and support, ensuring program adherence.
• Document all client communications accurately.
• Communicate with Primary Care Clinician or designee regarding patient needs and ensure care plans are complete.
• Keep Supervisor informed of issues related to the care plan process.
• Collaborate with cross-organizational teams to meet patient needs.
• Become skilled at using technology including secure email, telephone systems, and EMR.
• Adhere to documentation protocols and daily work logs.
