• Must be legally authorized to work in the United States.
• Must hold active, unrestricted clinical certification/licensure (MA/CMA, LPN, or RN).
• Minimum 1 year of patient-facing experience in long-term care or skilled nursing.

Responsibilities

• Coordinate care for long-term care residents using the organization’s EMR.
• Maintain regular on-site engagement to support care plans, follow-ups, and transitions.
• Collaborate with Ennoble Care providers within the long-term care setting.
• Serve as liaison between providers, facility staff, residents, and families.
• Document patient interactions and care coordination activities according to protocols.
