Responsibilities

• Plan and lead activities for residents across groups.
• Coordinate communication with residents, families, coworkers, and partners.
• Contribute ideas to the monthly activity calendar and track attendance.
• Support quality improvement alongside the Activities Director.
• Assist with discharge planning and create activity care plans.
• Coordinate transportation for outings and discharges.
• Develop diverse activities including outings and in-room options.
• Encourage hobbies and provide Braille books or audiobooks as needed.
• Assist with assessment documentation and progress notes.
• Keep the Activities Department clean, organized, and welcoming.
