Job summary

Pruitthealth Corporation in the United States is seeking an Activities Assistant HCC to plan, implement, and evaluate recreational and social programs aligned with each resident’s care plan.

You will work with an interdisciplinary team, conduct assessments, schedule activities across various times and locations, and adapt activities for residents with different abilities, including those confined to rooms.

Qualifications

• Requires minimum two years college education with courses in recreation, education, social work, other behavioral sciences or clinical services.
• Two years’ experience in a social or recreational program within the past five years, with one year full-time employment in a patient/resident activities program in a health care setting.

Responsibilities

• Assists in planning, implementation, and evaluation of recreation programs per patient care plan.
• Conducts assessments by interviewing residents, families and representatives to tailor programs.
• Updates recreation programs to reflect resident needs and rights.
• Works with interdisciplinary team and attends care conferences; communicates with staff.
• Schedules activities across morning, afternoon, evening, and weekend shifts.
• Provides activities for residents confined to rooms and for cognitively impaired residents.
• Documents progress notes and audits chart entries for accuracy.
• Participates in quality assurance and regulatory rounds; helps with policy updates.
• Communicates daily with Nursing Home Administrator on staffing and budget considerations.
