Honor Healthcare LLC (DBA Butterfly Home Care) in Manassas, VA seeks a Lead Registered Nurse to oversee in‑home care services for seniors and individuals with disabilities. You will provide clinical leadership, care coordination, and policy‑driven oversight to ensure safety, quality, and person‑centered planning across our care team.

Reporting to the Co‑Owner/CEO, you will collaborate with physicians and care staff, guide clinical decision‑making, supervise caregivers, and uphold Virginia

Qualificações

• Active RN or LPN license in Virginia in good standing.
• Clinical home care or disability services experience.
• Experience with seniors/disabled individuals and person-centered care.
• Strong nursing knowledge and safe caregiving techniques.
• Excellent communication and teaching skills.
• Ability to assess risk and respond to changes in condition.

Responsabilidades

• Provide clinical leadership for in‑home care services, ensuring safe, person‑centered, goal‑directed support.
• Oversee initial and ongoing assessments and develop individualized service plans.
• Monitor care delivery quality and drive improvements with leadership.
• Coach and supervise caregivers; provide training on key topics.
• Ensure documentation aligns with policies and Virginia regulations; manage risk.
• Collaborate with clients, families, and coordinators to support care needs.
• Promote teamwork and maintain a positive, compliant care culture.
