Responsibilities
- Provide clinical leadership for in-home care services and care coordination.
- Serve as primary clinical resource for caregivers, clients, and families.
- Coordinate with physicians, therapists, and case managers.
- Conduct or oversee initial and ongoing assessments and service planning.
- Monitor care quality and safety; implement improvements.
- Provide coaching and training to caregivers.
