Job summary

TheKey is seeking an LPN Supervisor to lead clinical oversight for Vienna, VA area clients. You will conduct telephonic intakes, in-person assessments, and update care plans, while supervising caregivers and ensuring regulatory compliance.

You will report to the Director of Nursing and travel locally 50–75% within Virginia, providing hands-on training and quality assurance in client homes.

Qualifications

• Current Virginia LPN license must be active.
• 3–5 years nursing in home care or related clinical setting.
• Ability to train and coach caregivers in the field.
• Strong customer service and communication skills for families and teams.
• Valid driver’s license with reliable transportation for local travel 50–75%.

Responsibilities

• Conduct nursing assessments in client homes and onboard new clients.
• Perform regular in-home supervisory visits and quality checks.
• Lead hands-on caregiver training and provide clinical mentorship.
• Address incidents and changes in client condition with care plan updates.
• Coordinate with scheduling to ensure best caregiver-to-client matches and accurate care plans.
• Maintain accurate client records and ensure regulatory compliance.
• Engage with local communities to support brand visibility.
