Job summary

Honor Healthcare LLC (DBA Butterfly Home Care) in Manassas, VA seeks a Lead Registered Nurse to oversee clinical leadership, care coordination, and quality across in‑home services for seniors and people with disabilities.

You will partner with the CEO and care teams to ensure person‑centered planning, regulatory compliance with Virginia standards, staff development, and safe, dignified care in clients’ homes.

Qualifications

• RN or LPN license in Virginia in good standing.
• Clinical experience in home care, disability services, or long-term care.
• Experience supporting individuals with disabilities and seniors with person-centered, community-based services.
• Strong nursing knowledge, safe caregiving techniques, and health/wellness promotion at home.
• Ability to assess risk, recognize changes in condition, and respond within scope.

Responsibilities

• Provide overall clinical leadership for in-home care services, ensuring safe, person-centered, and goal-directed support.
• Serve as primary clinical resource for caregivers, staff, clients, and families on health concerns and best practices.
• Coordinate care with physicians, therapists, case managers and other providers for integrated services.
• Oversee initial and ongoing assessments and develop individualized service plans.
• Monitor care quality with check-ins, home visits, and caregiver documentation reviews.
• Provide clinical guidance on complex care and changes in condition.
• Collaborate with leadership to support staff training and competency.
