Job summary

Mercy Rehabilitation Hospital, Northwest Arkansas in Rogers, AR is seeking a Case Manager for inpatient rehabilitation. This full-time, days role involves leading care coordination, discharge planning, and collaborative work with the multidisciplinary team to ensure high-quality patient outcomes.

Responsibilities include IPoC documentation per CMS guidelines, coordinating team conferences, and communicating progress to caregivers.

Qualifications

• Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT or SLP.
• Minimum 2+ years of case management or social work in an inpatient setting (rehab experience preferred).
• Strong communication, organization, and collaboration skills.
• CCM/ACM/RN-BC/ARN certification preferred.

Responsibilities

• Lead interdisciplinary care coordination and discharge planning.
• Identify barriers to progress and help develop solutions with the care team.
• Document care activities and IPoC per CMS guidelines.
• Facilitate weekly team conferences and family meetings.
• Communicate patient progress and discharge plans to caregivers.
• Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities.
• Support additional departmental needs as assigned.
