Job summary

Mercy Rehabilitation Hospital, Northwest Arkansas, is seeking a Case Manager for inpatient rehabilitation. The role focuses on coordinating interdisciplinary care and discharge planning to ensure seamless patient transitions.

You will identify barriers, document IPoCs, and lead family meetings while collaborating with the care team. Strong communication and CMS guideline proficiency are essential for success in this role.

How you’ll contribute

• Lead interdisciplinary care coordination and discharge planning
• Identify barriers to progress and help develop solutions with the care team
• Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
• Facilitate weekly team conferences and family meetings
• Communicate patient progress and discharge plans to caregivers
• Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
• Support additional departmental needs as assigned

What we’re looking for

• Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP
• 2+ years of case management or social work experience in an inpatient setting (rehab experience strongly preferred)
• Strong communication, organization, and collaboration skills
• Proficiency in Microsoft Office
• Case Management or Rehab Nursing certification (CCM, ACM, RN‑BC, ARN); preferred
