Qualifications

• Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT or SLP.

Responsibilities

• Lead interdisciplinary care coordination and discharge planning.
• Identify barriers to progress and help develop solutions with the care team.
• Document care activities and complete the IPoC per CMS guidelines.
• Facilitate weekly team conferences and family meetings.
• Communicate patient progress and discharge plans to caregivers.
• Review CMGs and collaborate with HIM on comorbidities.
• Support additional departmental needs as assigned.
