Job summary

Mercy Health Rehabilitation Hospital in Youngstown is seeking a Case Manager, RN or Social Worker for a PRN role to coordinate patient care in an inpatient rehabilitation setting. You will complete IPoC per CMS guidelines, schedule family conferences, and collaborate with the care team to remove barriers to discharge.

The position requires RN or Social Work licensure, with experience in inpatient case management preferred.

Qualifications

• Bachelor's degree in social work or RN.
• Minimum of 2 years social work or case management experience in an inpatient setting; acute/rehabilitation hospital experience preferred.
• Current RN or Social Work licensure or healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred.
• Strong organizational and communication skills.

Responsibilities

• Completes departmental orientation, initial and annual competencies.
• Assists with departmental specific performance improvement initiatives collecting and reporting data as requested by supervisor.
• Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care and discharge planning.
• Completes documentation per workflow timeline and CMS IPoC guidelines.
• Schedules family conferences and/or communicates with caregiver after team conferences to update progress and discharge plans.
• Coordinates weekly patient care team conferences to refine treatment plans and monitor patient goals.
• Reviews patient CMG to identify missed comorbid conditions and communicates findings to the HIM team.
• If no Lead Case Manager, participates in national CM Conference calls and communicates updates to facility CMs.
• Assists with concurrent and retrospective utilization review activities including denials and appeals with physicians and payers.
• Performs intake assessment within 24 to 72 hours of admission and follow-up assessments per CM plan.
• Other duties as assigned.
