Job summary

Mercy Rehabilitation Hospital, Northwest Arkansas, operated jointly with Lifepoint Health, is seeking a Case Manager to coordinate care and discharge planning. You will lead an interdisciplinary team, identify barriers, document IPoCs per CMS guidelines, and communicate progress to families and caregivers.

Ideal candidates bring active licensure and 2+ years in inpatient case management or rehab settings, with strong communication and collaboration skills and proficiency in Microsoft Office.

Qualifications

• Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP.
• 2+ years of case management or social work experience in an inpatient setting (rehab experience preferred).
• Strong communication, organization, and collaboration skills.
• Proficiency in Microsoft Office.

Responsibilities

• Lead interdisciplinary care coordination and discharge planning.

• Facilitate weekly team conferences and family meetings.

How You'll Contribute

• Facilitate weekly team conferences and family meetings
• Communicate patient progress and discharge plans to caregivers
• Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
• Support additional departmental needs as assigned

What We're Looking For

• Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP
• 2+ years of case management or social work experience in an inpatient setting (rehab experience strongly preferred)
• Strong communication, organization, and collaboration skills
• Proficiency in Microsoft Office
• Case Management or Rehab Nursing certification (CCM, ACM, RN‑BC, ARN); preferred
