Job summary

The Senior Housing Supportive Services Specialist at AIDS Foundation Chicago coordinates HUD/HOPWA housing programs and serves as a liaison to partner agencies providing housing and direct services for chronically ill and low‑income individuals in Chicago. The role supports case managers, ensures program policies are followed, and tracks referrals and eligibility.

You will oversee data entry in Case Worthy/HMIS, generate monthly reports, and participate in program audits.

Qualifications

• Bachelor's degree and 2+ years of Human Services experience
• OR 5+ years of Human Services experience
• Master’s degree in Psychology, Social Work, Counseling or related field

Responsibilities

• Coordinate HUD/HOPWA housing programs and liaise with partner agencies
• Provide oversight to supervisors and case managers in accordance with program policies
• Utilize Coordinated Entry System (CES) to track project data and referrals
• Monitor service utilization and report outcomes to AFC staff and partner agencies
• Lead meetings and trainings for partner agencies and new staff
• Ensure HMIS data standards and program audits are maintained

Education
