Job summary

Westside Federation of Senior & Supportive Housing in The Bronx, NY seeks a Social Worker I to build 1:1 resident relationships and provide a broad range of social work services for safe, independent living in the community.

The role supports MSW-level staff pursuing licensure, with study groups, licensing fee coverage, and weekly supervision by a LCSW. After licensure, CEU funding helps maintain licensure; full-time with benefits.

Qualifications

• MSW or MA in human services field with a minimum of two years social work experience.
• Fluency in written and oral English required; bilingual a plus.
• Experience with older adults and people with mental illness and/or substance use preferred.

Responsibilities

• Assist residents with obtaining and maintaining appropriate medical and mental health services, entitlements and home care.
• Establishing and/or maintaining family and social relationships
• Budgeting, income maintenance etc. to ensure on-time payment of rent
• Money management
• Crisis intervention services as needed
• Client appointment escorts as needed
• Individual counseling
• Through in-house and community referrals, assist residents in participating in socialization activities
• Follow up with resident and other service providers during hospitalizations or short-term rehabilitation placements
• Oversee discharge planning process and complete necessary forms
• Provision of group work services for residents
• Participate in the intake process of prospective residents
• Share information about resident progress with staff per confidentiality policy
• Complete incident reports as assigned
• Maintain competency in software and databases
• Attend supervision and team meetings
• Maintain thorough case records
• Other duties as assigned
