Job summary

Westhab, Inc in the Bronx– NYC is hiring a Social Worker/Client Care Coordinator (LMSW) to assess psychosocial functioning, coordinate mental health and substance abuse treatment, connect clients to wellness services, provide crisis intervention, and manage a caseload of high‑risk families in a families with children homeless shelter.

The role requires a master’s in social work (CSWE‑accredited) with LMSW eligibility within 6 months, at least two years’ experience, and Spanish language skills

Qualifications

• Must have a master’s degree in social work from a CSWE-accredited school.
• LMSW required within 6 months of hire.
• At least two years’ experience in a specialized social work practice.
• Fluency in Spanish is highly desirable.

Responsibilities

• Assess the psychosocial functioning of clients.
• Coordinate mental health and substance abuse treatment for clients.
• Connect clients to wellness services and crisis intervention as needed.
• Maintain and manage a caseload of high-risk families in a homeless shelter.
• Meet with clients individually and in groups to develop and implement treatment plans.
