Qualifications

• Knowledge of Fair Housing laws and regulations.
• Knowledge of HUD program requirements for 202s and PRACs.
• Knowledge of local service agencies and resources for the elderly, knowledge of entitlement programs for the elderly
• Understanding of legal liability issues relating to providing service coordination
• Demonstrated working knowledge of working with the elderly and knowledge of the process and issues of aging, including disability, mental health and abuse issues

Responsibilities

• Identify human service needs of residents and develop service plans.
• Identify available community services and arrange on-site or referral access.
• Educate residents and staff on aging in place and service provision.
• Assist the Manager with resident independence issues.
• Travel 5–20% to support residents and coordinate resources.
