Job summary

City Care Inc. in Tulsa is seeking an organized Housing Navigator to help single adults transition into safe, affordable housing within a psychosocial rehabilitation framework. This role emphasizes client-centered planning and collaboration with partners to achieve stable housing.

You will lead individualized housing plans, document progress, and coordinate with local landlords, Continuum of Care, and community agencies. A valid Oklahoma driver's license and reliable transportation are required.

Qualifications

• Previous experience in case management, recovery, or mental health services.
• Computer literate, resulting in error-free data entry.
• Excellent written and oral communication and partnership-building abilities across sectors and disciplines.

Responsibilities

• Well-versed in community resources, developing a network of collaborative partners
• Develop housing individualized plans with guests based on their self-directed goals
• Improve guests’ outcomes by collaborating to develop, monitor, and evaluate service plans
• Provide case management assistance, resource development, crisis intervention and direct support to individuals in the shelter (including document recovery, collaboration with the City of Tulsa's Continuum of Care and other partner agencies to leverage resources and facilitate progress)
• Promote housing stability for each resident by documenting progress, referrals, and contacts in the electronic record. Ensure ROI’s, case notes, and weekly documentation is updated and in compliance with best practices
• Provide leadership, case consultation and oversight to Housing Navigation team at The Harbor while also managing individual caseload
• Educate new incoming guests about rules and policy
• Engage in weekly face-to-face meetings with guests as directed
• Attend weekly staffing and community meetings as directed
• Manage and refine systems for tracking and improving housing outcomes and quality data retention at The Harbor.
• Build and maintain relationships with local landlords, housing authorities, Continuum of Care partners, and community-based organizations to expand housing options and streamline access.
• Uphold professional confidentiality among guests, interns, externs, volunteers and staff
