Job summary

City Care Inc. is seeking a Housing Stability Advocate to join a collaborative team serving residents recently transitioning out of homelessness.

You will conduct home visits, develop individualized service plans, and coordinate with partners to ensure housing stability and well-being. Requirements include case management or recovery services experience, disability experience, and a valid Oklahoma driver's license.

Qualifications

• Experience in case management or recovery services.
• Experience with adults and/or children with disabilities.
• Pre-employment background check and driving record required.
• Current Oklahoma driver's license and reliable transportation.
• Ability to enter data in HMIS.

Responsibilities

• Carry out City Care's mission, vision, and values through housing stability work.
• Conduct home- and community-based visits to support housing stability and well-being.
• Develop individualized service plans with residents and monitor progress.
• Coordinate with City Care programs and external partners to identify housing candidates.
• Provide crisis intervention, advocacy, and direct support based on resident needs.
• Document contacts, referrals, and service plans in accordance with requirements.
• Transport residents for appointments and resources as needed.
