Job summary

City Care Inc. is hiring a Housing Navigator to assist individuals in the homelessness response system transition into safe, affordable housing.

The role focuses on meeting guests’ goals within a psychosocial rehabilitation framework, developing individualized housing plans, and coordinating with a network of community partners. You will track progress, maintain documentation in electronic records, provide crisis support, and help expand housing options.

Qualifications

• Experience in case management, recovery, or mental health services.
• Computer literate with accurate data entry.
• Belief in housing as a human right and housing-first approach.
• Must have Oklahoma driver’s license, reliable transportation, and current insurance.
• Strong written and oral communication and partnership-building abilities across sectors.

Responsibilities

• Develop housing plans with guests based on their goals.
• Improve guest outcomes by monitoring and evaluating service plans.
• Provide case management, resource development, and crisis intervention.
• Document progress, referrals, and contacts in the electronic record.
• Build relationships with landlords, housing authorities, and partner agencies.
• Lead and supervise Housing Navigation team and manage caseload.
