Job summary

Cabrillo Hospice seeks an experienced Hospice RN Case Manager to lead and mentor an interdisciplinary team, oversee patient care, and ensure Medicare regulatory compliance. This role emphasizes case management, supervision, and hands-on home-based care.

Expect extensive travel to multiple locations with 1-10 home visits daily, coordinating with the medical director to maintain high-quality end-of-life care. Proficiency in EMR and HIPAA practices is essential.

Qualifications

• Bachelor’s degree or higher in a health-related field.
• Minimum 3 years of clinical RN experience, hospice experience required.
• At least 1 year of supervisory or management experience in a hospice setting.
• Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards; JCAHO knowledge preferred.
• Proficiency with Outlook, Word, and Excel.

Responsibilities

• Lead hospice care team and supervise, mentoring to ensure competency and high standards of care.
• Coordinate patient and family care, providing direction to interdisciplinary team members.
• Oversee care plans, ensuring accurate evaluations, appropriate treatments, and optimal patient outcomes.
• Monitor regulatory compliance with Medicare and hospice standards; ensure readiness.
• Review and approve care plans in EMR with accuracy and Medicare COP adherence.
• Participate in quality assurance initiatives and drive performance improvements.
• Lead interdisciplinary team meetings with clinicians and medical director.
• Provide ongoing education and documentation support for hospice staff.
• Assist Director of Clinical Services in regulatory readiness and survey preparedness.
• Participate in on-call leadership to ensure 24/7 patient care oversight.
