Job summary

Cabrillo Hospice is seeking an experienced Hospice RN Case Manager to lead patient care, mentor interdisciplinary teams, and ensure regulatory compliance in hospice services. The role emphasizes case management, documentation accuracy, and 24/7 on-call leadership.

You’ll travel to patient homes across the region, coordinate care plans, and support families with compassionate, evidence-based end-of-life care. Strong Medicare knowledge and EMR proficiency are required.

Qualifications

• Bachelor’s degree or higher in a health-related field.
• Minimum of 3 years of clinical RN experience, with hospice experience required.
• At least 1 year of supervisory or management experience in a hospice setting.
• Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards.
• Proficiency with Outlook, Word, and Excel.

Responsibilities

• Lead, mentor, and supervise hospice care team to maintain high standards of care.
• Oversee care plans, evaluations, treatments, and patient outcomes.
• Monitor compliance with Medicare and hospice regulations.
• Review and approve care plans in EMR with accuracy and POI adherence.
• Drive quality improvement initiatives and participate in team meetings.
• Coordinate interdisciplinary team meetings with medical director.
• Provide education and guidance for hospice staff and orientation.
• Assist the DCS in regulatory readiness and survey preparation.
• Participate in on-call rotation to ensure 24/7 patient care oversight.
