Job summary

Cabrillo Hospice is seeking an experienced Hospice Registered Nurse Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices. This role emphasizes leadership, case management, and regulatory excellence in home health settings.

You will lead an interdisciplinary team, manage care plans, and maintain Medicare compliance while delivering compassionate care to patients and families in diverse home environments.

Qualifications

• Bachelor’s degree or higher in a health-related field and active RN license.
• Minimum 3 years of clinical RN experience with hospice experience required.
• At least 1 year of supervisory or management experience in hospice.

Responsibilities

• Lead hospice care team and mentor staff.
• Oversee care plans and ensure Medicare compliance.
• Coordinate patient, family and interdisciplinary team.
• Monitor care quality and regulatory readiness.
• Document care plans in EMR accurately.
• Lead team meetings with medical director.
• Assist DCS with regulatory readiness.
• Participate in on-call leadership.
