Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an alternate infusion suite, under a physician's plan of care. The role requires adherence to safety, regulatory standards, and structured supervision.

The nurse will evaluate patient progress, administer therapies, educate families, and collaborate with the clinical team to ensure continuity of care and timely documentation for billing and compliance.

Qualifications

• Licensed registered nurse (RN).
• Experience coordinating and providing intermediate direct patient care in the home or alternate infusion setting.
• Ability to follow physician plans of care and supervise others as needed.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit and communicates changes to the physician.
• Provides training and mentorship to Infusion Nurse I's as needed to ensure patient safety and compliance.
• Provides patient education to achieve independence with prescribed therapy and care needs.
• Communicates with clinical team, providers, patients, and families to facilitate continuity of care and documents in real time for billing timelines.
• Coordinates discharge planning and reports patient care needs and progress on transfer.
• Ensures compliance with agency policies, procedures, standards, regulations, and accreditation requirements.
• Maximizes efficiency using computers to validate plan orders and document care.
