Advocate Health in Milwaukee, WI is seeking a Registered Nurse for the Post Acute Care Advanced Care program. The role focuses on delivering nursing care across home and community settings and coordinating with the ACAH team to support patient needs.

You will educate patients and families, document in the electronic health record, and participate in quality improvement activities, while traveling within the ACAH service area as required.

Formación

• Typically requires 2 years of experience in home care nursing or 3-4 years of medical surgical experience.
• Licensed RN in the state where practice occurs.
• Proficient in patient care equipment and documentation.

Responsabilidades

• Develops and maintains therapeutic relationships across the healthcare continuum with APC and ACAH teams.
• Assesses patient status and adjusts plans of care to optimize health and home care goals.
• Educates patients and families on disease processes and management strategies.
• Documents in the electronic health record per standards and policies.
• Participates in quality and safety initiatives and coordinates referrals as needed.
