Advocate Health is seeking a Nurse Case Manager in Wisconsin to oversee caseloads in specialty areas, perform initial assessments, and develop plans of care with physician orders. You will provide direct patient care, update plans based on patient response, and collaborate with the health care team to ensure smooth transitions at discharge.

The role requires an Associate Degree in Nursing with 3 years of clinical nursing experience, BLS where required, and active RN licensure.

Qualifications

• Associate Degree in Nursing required.
• RN licensure in state of practice.
• 3 years nursing experience typical for role.
• BLS certification within 6 months where required.
• Strong communication and organizational skills.

Responsibilities

• Assumes caseloads in specialty areas and conducts initial assessments.
• Establishes care plans per physician orders and patient needs.
• Provides direct patient care and adjusts nursing care plans as needed.
• Documents visits, assessments, and orders accurately and timely.
• Communicates with physicians and care team to update plans of care.
• Facilitates discharge planning and continuity of care after discharge.
• Participates in chart audits and quality improvement activities.
• Maintains accurate clinical records per guidelines.
