Health Wealth Safe, Inc. is seeking an in-home nursing case manager to deliver comprehensive case management for patients enrolled in the Advanced Care at Home program.

The role focuses on assessing clinical status, coordinating care plans, and connecting patients with community resources in a home-based setting. You will monitor treatment progress, escalate concerns as needed, and educate patients and families on care plans and self-management while maintaining clear communication with the care

• Strong clinical assessment and patient monitoring skills.
• Effective care coordination and referral management.
• Clear communication with patients, families, and healthcare providers.
• Ability to educate and support patients and caregivers.
• Proficiency with electronic health record documentation.

Responsabilidades

• Assess patients’ clinical status and care needs.
• Coordinate care plans and connect patients to resources through referrals.
• Monitor treatment response and progress, escalating concerns as needed.
• Educate patients and families on care plans, treatment goals, and self-management.
• Communicate patient status and care changes to the care team and ordering providers.
• Document assessments, interventions, and outcomes in the electronic health record.
