Responsibilities

• Develop and maintain a therapeutic relationship across the healthcare continuum with APC and ACAH teams.
• Assess patient’s clinical status and determine care needs or referrals.
• Plan, coordinate, monitor and evaluate care to optimize health and home life.
• Educate patients and families about disease processes and management.
• Enter and update documentation in the EHR per standards and policies.
