Job summary

Advocate Aurora Health in Milwaukee, WI is seeking a registered nurse to provide Home Health care within the ACAH program. You will build therapeutic relationships, assess clinical status, coordinate care, and refer to community resources to support the patient’s ability to remain at home.

You will educate patients and families, document in the electronic health record, and collaborate with the interdisciplinary team to optimize health outcomes.

Qualifications

• Bachelor's Degree in Nursing.
• Typically requires 2 years of experience in home care nursing or 3-4 years of medical surgical experience.
• Basic Life Support (BLS) certification required.
• Valid driver’s license.
• Registered Nurse license issued by the state.

Responsibilities

• Develops and maintains a therapeutic relationship with patients across the healthcare continuum.
• Assesses patient’s clinical status in person and facilitates referrals to community resources.
• Implements and coordinates plan of care to optimize health, quality of life, and independence at home.
• Educates patients and families on disease processes and management.
• Performs case management and participates in quality and safety initiatives.
• Documents patient care in the electronic health record and communicates changes to providers.
