Job summary

Tufts Medicine Care at Home in Winchester & Medford, MA is seeking a Home Health Registered Nurse to deliver comprehensive nursing services in patients’ homes. The role involves assessing, planning, implementing, and evaluating care per physician orders and the patient’s plan of care, with emphasis on safety and quality outcomes.

Responsibilities include coordinating with families and the health care team, documenting care activities, and ensuring timely medication reconciliation.

Qualifications

• Active RN license in the state of practice.
• Valid state Driver’s License.
• One year of acute medical/surgical nursing experience.
• Basic Life Support Certification.
• If hired prior to 9/28/2026, CPR Certification is required upon hire.
• If hired after 9/28/2026, BLS Certification is required within 30 days of hire.

Responsibilities

• Assesses patients' physical and psychosocial needs following Standards of Nursing Practice and Agency policies.
• Continuously assesses/evaluates patient and family progress and goals with the care team.
• Documents patient data, activities, and communications in patient records in a timely, organized manner.
• Completes physicians' orders, levels of care, and OASIS on all patients assigned; reconciles medications.
• Coordinates care and discharge planning with other team members and community resources.
• Provides nursing coverage on a rotating schedule including evenings, nights, weekends and holidays.
• Demonstrates knowledge of pharmacology and medication administration and reconciliation.
• Communicates and collaborates with all disciplines in the home care setting for critical needs.
